Almost everything you need can be purchased at home depot. I would suggest getting the foam from a carpet and flooring place though (end of the roll, etc), the material is much better (its not as hard). You can also get the felt here if you like.. i strongly suggest throwing cards across whichever felt you choose.. so bring some cards with you. Some of the nicest feeling stuff doesnt allow the cards to slide at all.
Go with the cheap carpet underlay, Ive built over 15 tables and experimented with probably more than 5 materials and I find the cheap stuff (just looks yellow, kinda like the extra cushion stuff you can buy for beds) works the best.
I never bothered to put cup holders, after all the work you do, you dont want someone spilling thier drink on the table. Go for mini stools, and counter sink cup holders into them.
